Job summary

PA Health Management in York, PA is seeking a CHC Service Coordinator to support Medicaid Waiver participants through a hybrid model of field and remote work. You will manage caseloads, develop person-centered plans, and coordinate services with providers to meet participant goals.

Required: a Bachelor’s degree in a related field and at least 3 years of service coordination experience; proficiency with Microsoft Office and electronic documentation systems; valid driver’s license and willingness

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field required.
• Equivalent combination of education and relevant experience may be considered.
• Minimum 3 years of experience in service coordination or related role preferred.
• Experience with waiver-funded or I/DD populations strongly preferred.
• Proficiency in Microsoft Office Suite and Teams; ability to work with multiple electronic documentation systems.
• Fluency in English; valid driver’s license; willingness to regional travel.

Responsibilities

• This role ensures timely, compliant, and person-centered service coordination for assigned participants.
• Independently manages caseload while collaborating with interdisciplinary teams to promote wellbeing and continuity of care.
• Conduct person-centered assessments and develop care plans at least quarterly or as needed.
• Coordinate services with healthcare providers, community agencies, and government entities.
• Monitor needs, outcomes, and service effectiveness; maintain accurate timelines.
• Participate in meetings, trainings, and interdisciplinary coordination activities.
