Job summary

Qualifications

• Experience with waiver-funded or I/DD populations strongly preferred.
• Proficiency in MS Office and data/documentation accuracy.

Responsibilities

• Coordinate services across Medicaid Waiver programs, MA State Plan, and other resources.
• Develop, implement, and update individualized service plans quarterly or as needed.
• Maintain accurate documentation and regulatory compliance for participant files.
• Engage participants through regular visits and remote/field-based support.
• Collaborate with providers, agencies, and interdisciplinary teams to meet goals.
