Job summary

PA Health Management is seeking a CHC Service Coordinator to support participants in accessing and coordinating services through Medicaid Waiver programs and other resources. The role combines remote administrative work with field-based participant support in a hybrid schedule.

The position requires a bachelor’s degree in a related field, at least 3 years of experience in service coordination, and fluency in English.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field required.
• Minimum of three (3) years of experience in service coordination or case management preferred.
• Experience with waiver-funded or I/DD populations strongly preferred.
• Fluency in English required.
• Valid driver’s license and reliable transportation for regional travel.

Responsibilities

• Coordinate services and monitor participant outcomes across home and community-based settings.
• Develop, implement, and update individualized service plans at least quarterly.
• Assist participants in identifying and accessing authorized waiver, medical, and social services.
• Coordinate with healthcare providers, community agencies, and government entities.
• Monitor participant needs, outcomes, and service effectiveness.
• Enter all required documentation accurately within established timelines.
