Qualifications

• Bachelor's degree in a Human Services field is required.
• One year of related experience is preferred.
• Licensure and clearances are required or preferred as listed.

Responsibilities

• Advocate for individuals to access necessary services.
• Ensure monthly contact and provide comprehensive service coordination.
• Facilitate service planning meetings.
• Communicate expectations and ensure service coordination.
• Assess strengths and needs through interviews and reviews.
• Evaluate services and review cases with relevant parties.
• Develop and maintain relationships with individuals and their support networks.
• Serve as the primary resource for individuals and families.
• Attend training programs for best practices and participate in projects.
• Coordinate mental health services as part of a treatment team.
• Promote collaborative service planning and maintain a catalog of community resources.
• Encourage recovery and community inclusion.
• Keep accurate records of activities and provide culturally competent services.
• Direct service should comprise at least 55% of time.
• Participate in On-Call Service.

Education

Bachelor's Degree in a Human Services field such as Social Work, Psychology or Counseling
