Job summary

Alleghenies United Cerebral Palsy is seeking a Service Coordinator to collaborate with clients to develop and manage personalized service plans. This role emphasizes client-centered support and requires strong communication and coordination skills.

The position offers an hourly rate of $21 with a generous benefits package, including flexible scheduling, PTO days, and tuition reimbursement. This hybrid role is intended for candidates residing in Pennsylvania, requiring both remote and sometimes in-person work.

Qualifications

• Expertise in Medicaid Waiver programs, LTSS, and HCBS is preferred.
• Reliable high-speed internet access is required for remote work.

Responsibilities

• Develop individualized service plans (ISP) with clients and teams.
• Update service plans to reflect life changes and needs.
• Monitor services to ensure appropriateness and compliance.
• Maintain open communication with support teams regarding updates.
• Ensure compliance with regulations and AUCP standards.
• Travel within the assigned region for home visits.
