Qualifications

• Bachelor's degree in a relevant field is required.
• At least 2 years of professional experience in case management for people with physical disabilities.
• Experience working with individuals with Autism is essential.
• Knowledge of Medicaid Waiver programs and Long-Term Services is preferred.

Responsibilities

• Collaborate with clients to develop individualized service plans based on personal goals.
• Update service plans to reflect life changes and current support needs.
• Monitor services to ensure they meet individual needs and are implemented properly.
• Maintain open communication with support teams.
• Ensure compliance with program regulations and documentation standards.

Skills

Experience working with individuals with disabilities or special needs

Strong skills in assessment, planning, and participant‑centered service delivery

Excellent organizational, communication, and documentation skills

Ability to work effectively as part of an interdisciplinary team

Familiarity with community support systems

Education

Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field
