Job summary

Froedtert Health in Wisconsin is seeking a Nurse Transfer Coordinator to facilitate patient transfers among Froedtert facilities and community hospitals. You will gather pertinent clinical data, coordinate level of care and transport mode, and document all transfer details in EPIC.

You will work with physicians, nursing, admitting, and patient flow teams to ensure timely placement and optimal occupancy.

Qualifications

• Minimum 4 years of nursing experience, with at least 3 years in acute care.
• Critical care or ED experience as an RN preferred.

Responsibilities

• Facilitate transfers to Froedtert Hospitals and community locations.
• Collect and review clinical information for transfer decisions.
• Determine level of care and transport mode with accepting physicians and providers.
• Document transfer discussions in EPIC and coordinate with hospital teams.
• Collaborate with physicians, nursing, admitting, and patient flow to ensure timely transfers.
