Qualifications

Responsibilities

• Coordinate and deliver patient care in ambulatory clinics.
• Provide direct patient care and patient education.
• Handle telephone triage and eVisits as part of care delivery.
• Assist with in-clinic minor procedures and care coordination across settings.
• Collaborate with interprofessional team to meet safety and quality goals.
