Job summary

LifeLong Medical Care is seeking a Registered Nurse II - Triage for a 40-hour weekly hybrid/remote role in the Bay Area. The RN will provide telephone triage care, document patient information in EPIC, and coordinate with the primary care team for timely follow-ups in Richmond, CA.

Qualifications include CA RN license and BLS, with an ADN/BSN. Prior nursing experience, strong communication, and teamwork are essential to support diverse patient needs in a federally qualified health center

Qualifications

• Current, unencumbered California RN license.
• BLS certification.
• Associates or Bachelor’s Degree in Nursing.

Responsibilities

• Provide patient care via telephone triage and MyChart messages.
• Coordinate with primary care team for follow-up and care transitions.
• Order labs and preventive screenings per clinician-approved protocols.
• Support care plans, ensure patient self-care goals are met.
• Serve as a resource for support staff and facilitate follow-up.
