Job summary

LifeLong Medical Care is hiring a Registered Nurse II - Triage in Richmond, CA for a part-time, 20 hours per week hybrid/remote role. The RN will perform telephone triage and coordinate with the primary care team using EPIC and MyChart messaging.

California RN license and ability to attend in-person orientation in Oakland are required. Salary ranges from $51.00 to $59.13 per hour, with benefits and a union through SEIU-UHW.

Qualifications

• Associates or Bachelor’s Degree in Nursing.
• California RN license in good standing with BLS.
• Experience coordinating care in a clinical setting is preferred.

Responsibilities

• Triage and provide patient care via telephone and MyChart messaging.
• Follow up with patients and coordinate with primary care teams.
• Support Care Transitions for hospitalized patients to ensure timely follow-up.
