Job summary

LifeLong Medical Care is seeking a Registered Nurse II - Triage to support patient care via telephone triage and MyChart messaging. The role is part-time (20 hours/week) with hybrid/remote options, requiring CA RN licensure and in-person orientation in Oakland with training in Richmond as needed.

Qualifications

• Current, unencumbered California RN license.
• Ability to coordinate care with triage assistants and support staff.
• Strong organizational, administrative and problem‑solving skills.
• Excellent interpersonal, verbal, and written communication skills.

Responsibilities

• Triage patients via telephone and MyChart messages.
• Follow up with patients and ensure communication with the primary care team.
• Coordinate with Care Transitions for hospitalized patients to ensure timely follow-up.
• Use approved procedures to order labs and preventive screenings and refill routine medications.
• Collaborate with team to meet patient self-care goals and psychosocial needs.
