Qualicare Home Care Central NJ is seeking a compassionate Registered Nurse (RN) for home health care on a per-visit basis in New Jersey. The role includes in-home client assessments, care plan development, supervision of CHHAs, and follow-up visits to ensure high-quality care.

Qualifications

• Active RN license in New Jersey (Required).
• 1+ years nursing experience, preferably in home care or case management.
• Current BLS/CPR certification (Required).
• ACLS and/or PALS certification (Preferred).
• BSN preferred; MSN is a plus.

Responsibilities

• Perform initial and ongoing physical and mental health assessments in clients' homes.
• Develop, implement, and update individualized care and service plans.
• Provide education on disease management, medications, self-care, and home safety.
• Supervise and train CHHAs; deliver performance feedback for safe, effective care.
• Conduct supervisory visits to monitor care quality and adherence to plans.
• Identify, document, and report incidents or changes per agency protocols.
• Maintain accurate documentation of visits, assessments, trainings, and evaluations.
• Ensure services comply with New Jersey State home care regulations and agency policies.
• Support agency launch through operational development and community outreach.
