Job summary

ComforCare HomeCare is seeking a compassionate RN Assessment Nurse to perform in-home assessments, create individualized care plans, and supervise HHAs/CNAs to ensure safe, high-quality care in clients' homes across Hunterdon, NJ.

You will collaborate with physicians and families, document accurately in the EHR, comply with NJ home health regulations, and support ongoing patient education and care coordination within a patient-centered framework.

Qualifications

• Associate Degree in Nursing (ADN) required.
• Bachelor of Science in Nursing (BSN) preferred.
• Active RN license in New Jersey with BLS/CPR certification.

Responsibilities

• Conduct in-home assessments for active and prospective clients.
• Develop, implement, and update individualized care plans.
• Supervise and evaluate Home Health Aides and CNAs.
• Coordinate with physicians and multidisciplinary team members.
