Job summary

Enlyte is seeking a Field Case Manager in the New Haven, CT area for a full-time, hybrid role. You will coordinate treatment plans with treating physicians, employers, and patients, ensuring cost-effective and effective care and a timely return to work.

Responsibilities include assessing recovery, advising on job adjustments, and maintaining clear communications with customers while independently managing a caseload. Travel to in-person appointments in assigned geography is required.

Qualifications

• RN license active and in good standing.
• 2+ years of clinical practice preferred.
• Workers’ compensation experience preferred.
• Proficient with basic computer tools (Excel, Word, Outlook).
• Ability to advocate recommendations with physicians, employers, and customers.

Responsibilities

• Collaborate with treating physicians and employers to create tailored treatment plans.
• Interview injured workers to assess recovery and occupational capability.
• Modify job duties with employers to facilitate early return to work.
• Provide status updates and submittals to customers and internal teams.
• Attend injured workers’ appointments and manage workload efficiently.
