Job summary

Genex Service LLC in Rochester, NY seeks a Medical Field Case Manager. This full-time hybrid role requires local travel for in-person appointments and collaboration with treating physicians, employers, and patients to develop effective treatment plans.

You will advocate for appropriate, cost-effective healthcare, monitor progress, adjust goals, and support safe return-to-work. A valid RN license and 2+ years clinical experience are preferred.

Qualifications

• RN license in good standing required.
• URAC-certified case management credentials preferred (CCM/CDMS/CRC/CRRN/COHN/CMAC, etc.).
• Must have reliable transportation and be able to travel to in-person appointments.
• Must have reliable internet access.

Responsibilities

• Create and implement individualized treatment plans with clinicians, employers and patients.
• Collaborate with physicians/providers to tailor treatment and return-to-work plans.
• Modify job duties with employers to facilitate early return to work.
• Manage cases independently, prioritizing workload and timely updates.
