Qualifications

• RN license in the state of practice or multi-state license via eNLC.
• Associate or Bachelor degree in nursing with required years of clinical experience.

Responsibilities

• Ambulatory utilization management and quality screening for assigned patients.
• Risk stratify patients and prioritize interventions.
• Collaborate with inpatient and outpatient case management to discharge planning.
• Coordinate transitions of care with physicians and families.
• Develop individualized care plans and coordinate services with the care team.
• Coordinate laboratory and diagnostic testing; monitor results and metrics.
• Participate in clinical pathways development and quality improvement.
