Qualifications

• Current RN license in the relevant state or multi-state license via eNLC.
• BLS certification required within 30 days of hire.
• Two pathways: ADN with 7+ years or BSN with 5+ years of clinical healthcare experience.
• State and federal background checks as required.

Responsibilities

• Manage ambulatory utilization, financial, and quality screening.
• Risk-stratify patient populations to prioritize needs.
• Coordinate discharge planning with inpatient and outpatient teams.
• Collaborate with physicians, patients, and families for safe transitions of care.
• Design individualized care plans and coordinate services.
• Coordinate financial assistance and entitlement referrals as needed.
• Lead clinical pathway education for staff and physicians.
• Use data to drive performance improvement and monitor outcomes.
• Oversee laboratory and diagnostic testing coordination.
• Support continuous quality and efficiency through metrics.
