Job summary

Commonwealth Care Alliance (CareSource affiliate) is seeking a Hybrid Registered Nurse Clinical Care Manager in Dorchester, MA. The role focuses on complex care management for dually eligible enrollees, coordinating healthcare and community resources, and reducing readmissions.

Qualifications

• ASN in nursing from an accredited program.
• Registered Nurse with MA licensure.
• Care coordination or dual‑eligible population experience preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in their homes and community settings to build complex care management relationships.
• Coordinate with ICT and other providers to develop holistic care plans.
• Oversight of long‑term services to keep enrollees in their preferred location.
• Educate enrollees and families about benefits and preventive care.
• Coordinate access to housing, transportation, food assistance and social services.
