Job summary

Commonwealth Care Alliance in Springfield, MA is seeking a seasoned RN Care Manager to provide in-home and community-based clinical care management for dually-eligible enrollees with complex needs. The role focuses on integrating health services and coordinating community resources to improve health outcomes and quality of life.

The RN works with the interdisciplinary team, conducts assessments, develops care plans, educates members and families, and travels to member homes and sites as required

Qualifications

• RN with license and degree required.
• Experience in care coordination or dual-eligible populations preferred.
• Medicare/Medicaid knowledge preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build a strong care management relationship.
• Coordinate with healthcare providers and community resources to ensure smooth transitions.
• Perform comprehensive assessments and develop care plans with ICT involvement.
• Educate members and families about benefits, preventive care, and adherence.
• Travel to member, provider and community-based visits as required.
• Following hospitalizations, ensure continuity of care and prevent readmissions.
