Job summary

Acentra Health is seeking an RN Clinical Assessor to join the team serving Gaston and Cleveland counties, NC. The role involves needs-based eligibility determinations for Medicaid-funded PCS and assessing level of care to help individuals remain at home or in community-based settings.

The position is hybrid, with remote work and field visits. Candidates must reside in designated NC counties and may travel up to 60 miles in a day, potentially 120 miles or more.

Qualifications

• RN license in NC or compact multi-state
• A valid driver’s license and reliable transportation
• 2+ years of experience in a home care setting

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Ensure PCS are provided on a needs basis based on unmet needs and clinical status.
• Maintain privacy and dignity of individuals receiving assessments.
• Complete assessments within established timeframes for all eligibility types (initial, annual, change of status).
• Interview beneficiaries, family members, and informal caregivers during assessments.
• Guide beneficiaries in selecting PCS providers and participate in service plan reviews.
