Job summary

Ponos Care is seeking an RN Case Manager for a hybrid role in the Northern VA area. The position involves telephonic case management, home-based infusion therapy, RPM, and LTSS coordination across multiple states. Responsibilities include comprehensive assessments, care planning, infusion administration, and care coordination with interdisciplinary teams.

Qualifications include a BSN, an active multistate RN license, CPR/BLS, and 2+ years of clinical nursing experience. CCM preferred.

• Willingness to travel to patient homes across regions.

Responsibilities

• Conduct comprehensive member assessments via telephonic outreach or in-home visits.
• Develop and implement personalized care plans across medical, behavioral health, and social needs.
• Administer infusion therapies and monitor patient responses.
• Coordinate services across care continuum with providers and community resources.
• Document all care activities in the EHR and ensure regulatory compliance.
