Qualifications
- Must reside in Illinois
- Travel up to 50-75% around Will, Kankakee and surrounding areas, IL
- 3-5 years clinical practical experience
- Willing to work from home and collaborate virtually
- Excellent analytical and problem-solving skills
- Effective communications, organizational and interpersonal skills
- Ability to work independently
- Strong computer skills including MS Word, Excel, Outlook, and PowerPoint
- Proficiency with standard corporate software applications
Responsibilities
- Coordinate and facilitate appointment scheduling and care coordination
- Assist members with accessing benefits and education
- Collaborate with members, providers, and community organizations
- Support care management tools and resources to improve outcomes
