The successful candidate will have an Illinois RN license, 3–5 years of clinical experience, strong communication skills, and proficiency with Microsoft Office and proprietary systems.

Qualifications

• Must reside in Illinois.
• 3–5 years clinical practical experience.
• Willing and able to travel up to 50–75% from home location in and around Will, Kankakee and surrounding areas, IL.
• Independent work from home using collaboration tools and virtual connections.
• Strong analytical, problem‑solving, and interpersonal skills.
• Proficiency with MS Word, Excel, Outlook and PowerPoint; familiarity with proprietary apps.

Responsibilities

• Coordinate case management activities to support member health outcomes.
• Assist with appointment scheduling and access to benefits and education.
