Job summary

Responsibilities

• Performs admission assessments and documents findings accurately and timely.
• Coordinates care for ambulatory services (infusion center, wound care, DME, PT/OT).
• Facilitates discharge planning with care team for patients transitioning to the next level of care.
• Communicates concerns and recommendations to the health care team as needed.
• Provides patient/family education on levels of care and required Medicare documentation.
