Qualifications

Responsibilities

• Perform face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge, coordinating with primary care providers.
• Identify gaps in care and barriers to discharge; implement improvement strategies.
• Collaborate with attending providers and care teams to ensure timely, coordinated care.
• Educate members on diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation accurately and promptly.
• Maintain infection control and patient privacy standards at all times.
