Virginia Commonwealth University Health Systems is seeking an RN Care Coordinator to support a multidisciplinary team in transitioning patients from hospital to home or other care settings. The role ensures safe, efficient care across the continuum with emphasis on discharge planning and payer collaboration.
Required Virginia RN license or eligibility and 3+ years in acute care; master's degree preferred. This full-time role offers day/evening shifts in the Richmond area.
To assist with coverage of full time staff PTO and weekend coverage.
The RN Care Coordinator functions as a part of a multidisciplinary team, including physicians, social workers, discharge planning assistants and payers. This position ensures that patient's progress in the acute episode of care through post discharge is quality driven while being efficient, cost effective and safe. This position will also be patient focused and outcome oriented.
Licensed Registered Nurse in the State of Virginia or eligible
Current RN licensure in Virginia Complete 15 Continuing Education Units per year
Minimum of three (3) years of nursing experience in an acute care setting
Baccalaureate Degree in Nursing from an accredited School of Nursing,or a Bachelor's degree in a biological, health or social sciences field from an accredited school.
Master's Degree in Nursing or a healthcare related field from an accredited program
