Responsibilities

• Facilitate care coordination from acute to ambulatory, as part of the healthcare team.
• Collaborate with Physicians, Case Managers, and other team members to coordinate care and follow-up appointments.
• Serve as a resource for care coordination and connect patients to post-acute and ambulatory options.
• Maintain knowledge of community resources, post-acute care, and ambulatory options to support patient needs.
