Qualifications

• Minimum of 3 years clinical nursing experience in an ambulatory/clinic setting.
• Care Management or Care Coordination experience preferred.
• Utilization management/quality management experience preferred.

Responsibilities

• Oversees and coordinates population management to improve clinical and financial outcomes.
• Develops care management programs addressing health care needs in partnership with physicians and patients.
• Identifies, coordinates, and case manages a panel of high-risk patients and oversees caregivers.
