RD Nutrition Consultants LLC is seeking a Registered Dietitian Nutritionist to provide nutrition care at a skilled nursing facility in San Bernardino, CA. This part-time hybrid opportunity combines onsite resident care with remote support from our dedicated team.

The role requires RD/RDN credentials, 1–3 years of experience, and a small weekly onsite visit in San Bernardino. Compensation is $40.00-$45.00 per hour, with remote work up to 50% of hours.

Qualifications

• Current RD/RDN registration required with California credentials.
• Degree in Dietetics/Nutrition from an accredited program (Bachelor’s or Master’s).
• Experience in skilled nursing or long-term care preferred.
• Strong clinical assessment, communication, and organizational abilities.
• Familiarity with EMR and interdisciplinary care planning.
• Ability to work independently with a part-time hybrid schedule.

Responsabilités

• Complete nutrition assessments and follow-ups for residents at nutritional risk.
• Develop, implement, and monitor individualized nutrition care plans.
• Monitor weight, intake, labs, and other nutritional indicators.
• Recommend therapeutic diets, oral supplements, and enteral support as indicated.
• Collaborate with nursing, dietary staff, and the care team to support resident health.
• Provide nutrition education and counseling to residents and families.
• Document accurately in the facility’s medical record system.
• Participate in care conferences and nutrition-related meetings.
