Job summary

RD Nutrition Consultants LLC is seeking a Clinical Registered Dietitian to join our team in a part-time, hybrid role (50% remote). The position involves in-person visits at the skilled nursing facility in Fremont, CA, with flexible scheduling.

Responsibilities include nutrition assessments, care plan development, education, and collaboration with the care team, while ensuring CMS and DOH regulations are followed.

Qualifications

• Bachelor's or Master's degree in Dietetics, Nutrition, or related field.
• Registered by the Commission on Dietetic Registration.
• Current state licensure/certification (if mandated by the state).
• Minimum 1 year clinical experience (preferred).
• Excellent communication skills for patient and team interaction.
• Ability to apply critical thinking to assess and improve nutrition plans.
• Proficiency in providing general nutrition interventions.
• Familiarity with state survey processes and nutrition care regulations.
• Competency in electronic charting systems.

Responsibilities

• Conduct comprehensive nutritional assessments on admissions and quarterly thereafter.
• Develop, implement, and monitor nutrition care plans.
• Provide nutrition education and counseling.
• Collaborate with physicians, nurses, therapists and interdisciplinary team.
• Ensure compliance with CMS/DOH regulations.
• Monitor weight changes weekly/monthly to identify nutrition issues.
• Participate in quality improvement to enhance care.
• Review and approve menus to meet USDA guidelines.
• Perform other tasks within scope of practice.
