Job summary

Qualifications

• Bachelor's or Master's degree in Dietetics, Nutrition from an accredited program.
• Registered by the Commission on Dietetic Registration.
• Current state licensure/certification as required.
• Minimum of 1 year clinical experience (preferred).
• Excellent communication with patients and healthcare teams.
• Able to apply critical thinking and develop care plans.
• Familiarity with charting systems.

Responsibilities

• Conduct comprehensive nutritional assessments.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition education and counseling.
• Collaborate effectively with physicians, nurses, therapists, and other members of the interdisciplinary care team.
• Ensure compliance with CMS and DOH regulations.
• Monitor residents' weight changes and nutritional status.
• Participate in quality improvement initiatives to enhance care.
