Job summary

RD Nutrition Consultants LLC is seeking a Clinical Registered Dietitian for a hybrid role with 50% remote telehealth and 18–26 hours per week. The position includes one on-site visit per week and flexibility to choose days and hours, with compensation ranging from $40.00 to $45.00 per hour based on experience.

This facility-focused role emphasizes nutrition assessments, care-plan development, and collaboration with the care team to ensure high-quality resident outcomes while maintaining CMS/DOH

Qualifications

• Bachelor's or Master's in Dietetics from an accredited program.
• Registered by the Commission on Dietetic Registration.
• Current state licensure/certification if mandated.
• Minimum of 1 year clinical dietetics experience.
• Excellent communication with patients and healthcare teams.
• Ability to apply critical thinking to develop and monitor care plans.
• Familiarity with state survey processes.
• Competency in electronic charting systems.

Responsibilities

• Conduct comprehensive nutritional assessments on new admissions and quarterly thereafter, with more frequent reviews for high-risk residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition education and counseling as needed.
• Collaborate with physicians, nurses, therapists, and other team members.
• Ensure compliance with CMS and DOH regulations.
• Maintain accurate documentation of care.
• Monitor resident weight changes weekly/monthly.
• Participate in quality improvement initiatives to enhance care and outcomes.
• Conduct monthly kitchen sanitation audits and document findings.
• Review and approve menus to meet USDA guidelines.
