Qualifications

• Bachelor's or Master's in Dietetics or related field, accredited program.
• Registered Dietitian with the Commission on Dietetic Registration.
• Active state licensure/certification if required by state.
• Minimum 1 year of clinical experience preferred.
• Excellent communication with patients, families, and care teams.
• Strong critical thinking and problem-solving for care plans.
• Proficient in general nutrition interventions and charting.

Responsibilities

• Conduct nutritional assessments on admissions and quarterly reviews.
• Develop, implement, and monitor nutrition care plans.
• Provide nutrition education and counseling as needed.
• Collaborate with physicians, nurses, therapists, and care team.
• Ensure compliance with CMS, DOH, and state regulations.
• Maintain accurate documentation and records.
• Monitor resident weight changes weekly or monthly.
• Participate in quality improvement to enhance care.
• Conduct monthly kitchen sanitation audits and report findings.
• Review and approve menus to meet USDA guidelines.
• Perform additional tasks within scope of practice.
