Job summary

Hidden Roots Mental Health LLC seeks a qualified Community Engagement / QMHP-CS for Medicaid-funded behavioral health services in the Greater Houston area. Hybrid role with ~70% billable client services and ~30% outreach, coordinating a small team and building partnerships with schools, shelters, clinics, and MCOs.

Responsibilities include intake assessments, documentation accuracy, compliance with Texas regulations, and supervision support to ensure quality care and program growth.

Qualifications

• Must meet Texas QMHP qualifications for the assigned services and program.
• Experience in mental health, case management, or community outreach.
• Leadership, team coordination, or supervisory experience preferred.
• Experience developing referral networks and community partnerships strongly preferred.
• Knowledge of HIPAA, documentation, and Texas requirements.

Responsibilities

• Conduct intake assessments and provide case management services within qualifications.
• Support onboarding, training, case coordination, and workflow concerns.
• Monitor team productivity, documentation, and compliance.
• Develop and maintain relationships with schools, shelters, hospitals, clinics, and partners.
• Track outreach activities and referral outcomes weekly.
