Qualifications

• High school graduate or equivalent; Associate Degree in PTA required.
• Ability to work independently with sound clinical and business judgment.
• Strong multitasking abilities and organizational skills.
• Valid driver’s license with a reliable insured vehicle.
• Flexibility to assist with on-call duty rotation.

Responsibilities

• Care Coordination: Develop and update comprehensive client care plans; communicate with clients, families, care partners, and medical sources.
• Clinical Oversight (PTA): Assess home safety, support mobility goals, and align with PT recommendations.
• Quality Assurance: Conduct in-home supervisory visits to ensure safe, high-quality care.
• Scheduling & Continuity: Manage and schedule client care needs for consistency.
• Office & Team Support: Maintain a calm, positive demeanor and assist with after-hours duties.
