Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) to deliver skilled visits in patients’ homes under PT supervision. You will focus on gait, balance, transfers, strengthening, and home safety, coordinating with the clinical team to ensure safe, measurable progress.

You will document in Homecare Homebase and participate in a productivity-based schedule designed to optimize field time and patient care.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field (PTA supervised).
• Comfortable escalating changes in condition to PT/RN/MD with clear rationale.
• Strong communication and documentation skills.
• Reliable follow-through on treatment plans and patient goals.
• Valid driver’s license with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment.
• Competency in core PTA home health scope including gait, transfers, safety.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on mobility and progress: gait/balance, transfers, strengthening.
• Set and track objective progress within PTA scope, communicate updates to supervising PT.
• Drive multidisciplinary coordination and keep care team aligned.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB) — defensible and consistent.
