Job summary

Qualifications

• Bachelor's degree required; licensed to practice pharmacy in New York.
• Five years of pharmacy experience required; specialty pharmacy preferred.
• Must maintain a current New York Pharmacist License and work independently.

Responsibilities

• Provide and coordinate pharmaceutical services for assigned patient population.
• Evaluate medications for appropriateness of therapy and drug interactions.
• Prepare patient records, care plans, and discharge planning.
• Ensure adherence to URAC and ACHC standards and state/federal laws.
