Job summary

Acentra Health, LLC is seeking a PASRR Clinical Evaluator - PRN to support PASRR evaluations and care level recommendations in the Sacramento area. The role blends telehealth and in-person assessments in nursing facilities and hospitals, with a flexible PRN schedule.

The ideal candidate holds an active California license in a relevant field, has 3+ years of mental health assessment experience, and 1+ year with developmental disabilities or related conditions.

Qualifications

• Active, unrestricted California licensure in healthcare or related field.
• Graduation from an accredited college or university with a degree in a relevant field.
• 3+ years of experience in the assessment/diagnosis of mental or psychiatric disorders, including complex differentials.
• 1+ years of experience with intellectual and developmental disabilities or related conditions.

Responsibilities

• Complete PASRR Level II assessments by reviewing records and collateral information.
• Develop level-of-care recommendations and determine appropriate next level of care.
• Review medical records and provide clinical recommendations per regulatory requirements.
• Interact with healthcare professionals, patients, families, and internal partners as needed.
• Utilize technology and software platforms to manage assessment activities.
• Ensure timely completion of assessments within scope of practice and contracts.
