Ocracoke Health Center, Inc. is seeking a Behavioral Health Case Manager to join our Integrated Care Team on a part-time, hybrid schedule with onsite days in Engelhard and Manteo.

The position focuses on referrals, care coordination, and addressing social determinants of health. Responsibilities include coordinating care with primary providers and the Behavioral Health Counselor, linking patients to specialty care and community resources, and maintaining compliant documentation in Athena.

Qualifikationen

• Bachelor’s degree in social work or related field required.
• One or more years of case management experience preferred.
• Experience with electronic health records (Athena a plus).
• Strong communication and collaboration with care teams.

Aufgaben

• Receive and manage referrals from providers and counselors.
• Coordinate care, assess, plan, follow up, and advocate for patients.
• Link patients to specialty care and community resources.
• Address social determinants affecting health (housing, transportation, food, finances).
• Collaborate with care team and document in the EHR (Athena).
