Job summary

Ocracoke Health Center, Inc. seeks a Behavioral Health Counselor to join the Integrated Care Program Lead role, providing behavioral health services within a Patient Centered Health Team at Engelhard Medical Center and Manteo Community Health Center.

This is a hybrid, part-time position (two to three days per week) with remote work and onsite days; there is potential to expand to full-time if funding allows, with a comprehensive benefits package when full-time.

Qualifications

• Master’s degree in social work, psychology, marriage and family therapy, counseling, behavioral health, or closely related field.
• Current North Carolina licensure as LCSW or LMFT or LCMHC.
• Strong communication, clinical assessment, documentation, crisis intervention, and care coordination skills.

Responsibilities

• Provide evidence-based behavioral health interventions within a primary care setting.
• Assess patients referred by primary care providers and develop patient-centered care plans.
• Use behavioral health screening and assessment tools to identify concerns, support triage, and measure outcomes.
• Collaborate with medical providers and other members of the care team to support patients with emotional, behavioral, and mental health needs.
• Refer patients to specialty behavioral health, case management, or community resources when appropriate.
• Maintain accurate and timely documentation in the electronic health record - we use Athena
• Participate in chart reviews, quality improvement initiatives, clinical guideline development, staff education, and Patient-Centered Medical Home collaboration.
• Provide guidance and supervision to the Behavioral Health Case Manager.
