Job summary

Ocracoke Health Center, Inc. is seeking a Behavioral Health Case Manager to join our Integrated Care Team on a hybrid, part-time basis (~20 hours/week).

The role coordinates referrals, connects patients to care, and addresses social determinants of health, working with the Behavioral Health Counselor and primary care providers. The position emphasizes flexible onsite days in Engelhard and Manteo, with Athena as the EHR).

Qualifications

• Bachelor's degree in social work or a related human services field.
• Associates degree candidates may be considered if expected to graduate within one year.
• One or more years of experience in case management preferred, particularly in a community-based or healthcare setting.
• Experience with and/or ability to learn and use electronic health record systems (Athena experience is a plus).
• Commitment to maintaining patient confidentiality and professionalism.
• Knowledge of community resources and human service systems across the lifespan.
• Ability to work collaboratively with patients, healthcare providers, community partners, and other members of the care team.

Responsibilities

• Receive and manage referrals from primary care providers and behavioral health counselors.
• Coordinate care and provide case management services, including assessment, care planning, follow-up, advocacy, and resource coordination.
• Connect patients with specialty care, behavioral health services, and community resources that support their health and well-being.
• Assist patients in addressing social determinants of health, including barriers related to housing, transportation, food access, financial stability, and other community support needs.
• Collaborate closely with medical providers, behavioral health counselors, and other members of the care team to support patient goals and improve outcomes.
• Provide administrative and care coordination support to the Behavioral Health Counselor and Integrated Care Team.
• Maintain accurate, timely, and compliant documentation in the electronic health record (we use Athena).
• Participate in team meetings, case reviews, and care planning activities.
• Deliver patient-centered, culturally responsive services that respect the unique needs and circumstances of each individual.
