Ocracoke Health Center, Inc. is seeking a Behavioral Health Therapist / Integrated Care Program Lead to provide behavioral health services as part of our Patient Centered Health Team.

This hybrid, part-time role includes scheduled onsite days at Engelhard Medical Center and Manteo Community Health Center, with remote work options. The position emphasizes integrating behavioral health within primary care, leading an expanding program, and supervising the Behavioral Health Case Manager.

Qualifications

• Master’s degree from an accredited program in social work, psychology, marriage and family therapy, counseling, behavioral health, or a closely related field.

Responsabilités

• Provide evidence-based behavioral health interventions within a primary care setting.
• Assess patients referred by primary care providers and develop patient-centered care plans.
• Use behavioral health screening and assessment tools to identify concerns, support triage, and measure outcomes.
• Collaborate with medical providers and other members of the care team to support patients with emotional, behavioral, and mental health needs.
• Refer patients to specialty behavioral health, case management, or community resources when appropriate.
• Maintain accurate and timely documentation in the electronic health record – Athena.
• Participate in chart reviews, quality improvement initiatives, clinical guideline development, staff education, and Patient-Centered Medical Home collaboration.
• Provide guidance and supervision to the Behavioral Health Case Manager.
