Ocracoke Health Center, Inc. is seeking a part-time Behavioral Health Therapist / Integrated Care Program Lead to deliver behavioral health services within a primary care team. The role offers a hybrid schedule with onsite days at Engelhard Medical Center and Manteo Community Health Center.

The position requires NC licensure (LCSW, LMFT, or LCMHC) and a master’s degree in a related field. Flexibility for rural coastal communities is a plus, with a focus on integrated care and team collaboration.

Qualifications

• Master’s degree in social work, psychology, marriage and family therapy, counseling, behavioral health, or closely related field.
• Active North Carolina licensure (LCSW, LMFT, LCMHC).
• Experience in integrated care, primary care, community health, or rural behavioral health is a plus.
• Leadership or supervisory experience preferred, but not required.

Responsabilités

• Provide evidence-based behavioral health interventions within a primary care setting.
• Assess patients referred by primary care providers and develop patient-centered care plans.
• Use behavioral health screening tools to identify concerns, support triage, and measure outcomes.
• Collaborate with medical providers and care team members to support patients with emotional and behavioral health needs.
• Refer patients to specialty behavioral health, case management, or community resources when appropriate.
• Maintain accurate documentation in the electronic health record (Athena).
• Participate in chart reviews, quality improvement, and Patient-Centered Medical Home collaboration.
• Provide guidance and supervision to the Behavioral Health Case Manager.
