Job summary

CareOregon, Inc. is seeking a Panel Coordinator to support members within a primary care medical home, working with patients and physicians to promote timely access to care and continuity of care.

This hybrid role requires 1–2 days in clinic in east Portland or Scappoose, Oregon. Responsibilities include coordinating services for assigned panels, tracking population health metrics, participating in quality improvement, and addressing gaps in care for complex patients.

Qualifications

• Minimum 2 years relevant experience in a healthcare setting.
• Preferred Minimum 2 years in outpatient primary care and/or the care and service of patients with chronic disease.
• Experience as a Certified Medical Assistant.

Responsibilities

• Serve as a communication link between member and clinic staff by gathering information from members.
• Independently identify members in the practice by running panel reports for diagnosis and preventive care needs.
• Review and evaluate member records to identify preventive services, diagnostics, and follow up needs.
• Coordinate services for all members in the panel, focusing on gaps in care and population health strategies.
• Communicate with members about preventive care needs and refer to appropriate clinical staff.
