Job summary

CareOregon is seeking a Population Health Program Manager to translate strategy into operational execution for Medicaid and Medicare populations. You will design, implement, and evaluate programs, services, and processes to improve health outcomes, equity, quality, cost, utilization, and member experience.

Qualifications

• Minimum 6 years of experience in population health, public health, health care management, quality improvement, or a related field.
• At least 2 years in project or program management.
• Demonstrated matrix leadership (no direct reports required).
• Experience leading cross-functional teams and managing complex initiatives.
• Experience achieving impact through program or project management.
• Experience using population health data to drive decisions and action.

Responsibilities

• Lead the design, implementation, and ongoing management of evidence-based population health programs, interventions, and processes.
• Provide matrix leadership to coordinate population health initiatives across internal departments and teams.
• Define program success measures and monitor performance using population health data; evaluate program effectiveness and drive improvements.
