Job summary

Required to have a Medical Assistant Certificate/Diploma and BLS certification at hire, with at least one year of MA experience in a medical home setting; bilingual English/Spanish is preferred.

Qualifications

• High school diploma or GED required.
• Medical Assistant Certificate/Diploma required.
• Current Basic Life Support (BLS) certification required upon hire and maintained.
• One year of medical assistant experience within medical home health preferred.
• Non-profit community health clinic experience preferred.
• Bilingual-English/Spanish preferred.

Responsibilities

• Uses the primary care medical home model to provide coordinated team care addressing current diseases.
• Provides panel management for preventive and health maintenance follow-ups for the maximum number of patients per day.
• Contacts new patients to welcome them, explain the medical home, and answer questions.
• Schedules patients with chronic conditions for follow-up appointments.
• Performs population management tasks (scheduling, recalls, portal callbacks).
• Makes outreach calls to patients missing appointments per guidelines.
• Implements actions to increase productivity (same-day slots, full provider schedules).
• Tracks progress of tasks in weekly supervisor meetings.
