Humana is seeking an Onsite Care Coordination Nurse for McLaren Greater Lansing Hospital (Lansing, MI). The role blends in-person facility work with hybrid remote coordination to support members at critical care transitions.

You will perform face-to-face assessments, lead discharge planning, identify care gaps, and collaborate with care teams to promote safe, timely transitions and better health outcomes.

Qualifications

• Current, unrestricted RN license in Michigan.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office and electronic medical records.

Responsabilités

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Support and educate on discharge planning from admission through post-discharge.
• Identify gaps in care and barriers to discharge; implement strategies.
• Collaborate with providers and care teams for timely, coordinated care and discharge planning.
• Provide education on diagnoses, medications, and preventive measures.
• Complete required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
