Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during transitions from hospital to home, conducting face-to-face assessments in assigned acute care facilities and leading care coordination.

This hybrid role combines in-person work at the Monroe facility with remote documentation, requires NC RN license and 3+ years acute care, and offers a comprehensive benefits package.

Qualifications

• Current, unrestricted RN license in North Carolina.
• Minimum 3 years of acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Proficiency with Microsoft Office and electronic medical records.

Responsibilities

• Conduct comprehensive, face-to-face assessments for newly admitted members in acute care facilities.
• Educate members and families on discharge planning and follow-up with providers.
• Identify gaps in care and barriers to discharge and implement strategies.
• Collaborate with providers and care teams to ensure coordinated discharge planning.
• Provide education on diagnoses, medications, and preventive measures.
