Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during hospital-to-home transitions at designated facilities. You will perform face-to-face assessments, identify gaps in care, and coordinate with providers to ensure safe, timely discharges.

This hybrid role combines in-person work at facilities with remote coordination and documentation, requires a Kentucky RN license, 3+ years acute care experience, and a focus on whole-person care.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience using Microsoft Office and EMR systems.
• Valid driver’s license and access to a personal vehicle.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at acute care facilities.
• Educate members and families on discharge planning from admission through post-discharge.
• Identify gaps in care and barriers to discharge; develop improvement strategies.
• Collaborate with providers and care teams to ensure coordinated care and discharge planning.
• Provide education on diagnoses, medications, and preventive measures to promote wellness.
• Complete documentation accurately and maintain patient privacy standards.
