Qualifications

• Current, unrestricted RN license in Kansas.
• Located within a 30–60 minute drive to The University of Kansas Hospital.
• Minimum of 3 years of acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience with Microsoft Office and electronic medical records.
• Valid driver’s license, dependable vehicle, and proof of insurance.

Responsibilities

• Conduct face-to-face assessments of newly admitted members at acute care facilities.
• Support discharge planning from admission through post-discharge and coordinate follow-up visits.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement strategies.
• Collaborate with providers, facility case managers, and other healthcare professionals for coordinated care.
• Provide education on diagnoses, medications, and preventive measures to promote wellness and reduce readmission.
• Complete all required documentation accurately and maintain patient privacy standards.
